Fall 2025 Hospital Safety Grade release marks the lowest amount of A grades ever reported for hospitals across the state

NOVI, MICHIGAN, November 13, 2025 – The Leapfrog Group today released its Fall 2025 Hospital Safety Grade, an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries and infections. Driven in the State of Michigan by the Economic Alliance for Michigan (EAM), the Grades provide the community with critical insights about local hospitals. Hospital Safety Grades are updated twice a year by The Leapfrog Group, an independent national nonprofit watchdog.
Michigan hospitals rank 35th in the nation in patient safety ratings, with only 17 out of 79 hospitals earning an A grade – the lowest ever recorded. Out of the remaining hospitals, 33 earned a B, 22 earned a C, four earned a D, and three earned an F. Hospitals receiving an A grade for five consecutive terms are Corewell Health Zeeland Hospital, Garden City Hospital, Henry Ford West Bloomfield Hospital, Munson Healthcare Cadillac Hospital, and Trinity Health Livingston in Howell.
According to Leapfrog, an estimated 160,000 lives are lost annually from avoidable medical errors, and over 50,000 lives could be saved every year if B,C, D, and F hospitals improved their patient safety grades to an A. “When hospitals make preventable mistakes, it’s patients and employers who end up paying the price — both in lives and in higher healthcare costs. Strengthening patient safety must be a priority, because every preventable error represents a tragedy and an avoidable expense,” said Bret Jackson, President and CEO of EAM.
How Michigan’s hospitals scored:
- Seventy-five percent of Corwell Health hospitals received an A or B grade (16 hospitals were graded).
- Three of the four Detroit Medical Center (DMC) hospitals scored an F.
- Henry Ford Health scored the most A grades (four hospitals). Seventy-five percent of Henry Ford Health hospitals received an A or B grade (12 hospitals).
- McLaren Health Care had 56% of its hospitals scoring an A or B grade (9 hospitals).
- The five hospitals of Munson Healthcare that were graded earned an A or B grade (100% A or B grade).
- MyMichigan Health had 83% of its six graded hospitals receive an A or B.
- Seventy-five percent of Trinity Health Michigan hospitals received an A or B grade (8 hospitals were graded).
“When hospitals fall short on safety, everyone pays the price — patients, employers, and our entire economy,” added Jackson. “We must do better. Safer hospitals mean stronger businesses and healthier communities.”
